Before continuing to complete this form, please understand:
1) We only take shar-pei from the following states: California, Nevada, Arizona, Colorado, New Mexico, Idaho, Washington & Oregon. We will consider shar-pei from Utah and Wyoming.
2) You also must be the legal owner of the dog, filling out our surrender form.
3) Make sure you have the following ready:
- Name and phone for your current veterinarian
- Date of last Rabies vaccine
- 2 photos of your dog emailed to adopt@peipeople.com
This form could take you 15 minutes or more to complete and cannot be saved part way through, so please be prepared with all information above and to tell us about your dog. Thank you!
